• Sponsor eligibility letter - this is required only if your sponsor is not a member of St. Bartholomew's Parish: 1/31/2010

• Letter to the Bishop: 5/1/2010

• All service evaluation sheets: 5/1/2010
• All spiritual encounter evaluations: 5/1/2010
  • Note: It is best to complete the evaluation sheet and turn it in right after the service or spiritual encounter. Don't wait until the last minute to finish these - you may not remember the details of what you did or experienced.
